This is a legally binding document. Please read carefully before signing.
I understand that I am volunteering my time and services for Mr. Bob’s Under the Bridge (MBUTB). My participation is voluntary and does not create an employment relationship.
2. Assumption of Risk
I understand that volunteering may involve physical activity, lifting and carrying items, transportation activities, exposure to weather conditions, illness, emotional stress, and interactions with individuals experiencing homelessness or crisis.
3. Release & Waiver of Liability
In consideration of being permitted to volunteer, I release and discharge Mr. Bob’s Under the Bridge, its officers, directors, employees, agents, contractors, and volunteers from claims or liabilities arising from my participation to the fullest extent permitted by Wisconsin law.
4. Indemnification
I agree to indemnify and hold harmless MBUTB and its representatives from claims, damages, costs, or expenses arising from my actions or omissions while volunteering.
5. Transportation
If I drive during volunteer activities, I certify that I have a valid driver’s license and automobile insurance as required by Wisconsin law.
6. Confidentiality & Release of Information
I agree to keep confidential any personal information learned through my volunteer service and will not disclose, photograph, record, or share information outside my volunteer role.
7. Photography & Media
I grant MBUTB permission to use approved photographs or recordings of me for educational, fundraising, and promotional purposes without compensation.
8. Governing Law & Acknowledgment
This agreement is governed by the laws of the State of Wisconsin. I acknowledge that I have read and understand this document.
